Saint Pius X Awards Banquet Registration Form 2012 On the form below please list the following people in order: Any one who has completed ten years of volunteer service to catechetical ministry (Pius X Award) Any one who has completed 25, 30, 35, 40, 45 or more years of volunteer service to catechetical ministry (Special Award) Any one who will attend the St. Pius X Dinner (Present and past recipients, parish staff members, and spouses; priests) Please include a brief statement for anyone who will receive an award for 25, 30, 35, 40…years. As always, our goal is to keep the cost of the banquet as low as possible while providing an event worthy of those being honored. The price of dinner is $38.00 per person. The event is on May 31st at 7pm. Registration Deadline:  April 13th and No refunds after May 1st.
